A. A Senior High School Graduate from a CHED and DepED recognized institution.
B. Other additional Requirements as prescribed by the department:
(a) Interview
(b) SHS Card/ Form 137A

(c) 2 pcs 2x2 pictures and 2 pcs 1x1
pictures
(d) 1pc Brown Envelope
(e) PSA Birth Certificate
